Advance Care Planing in West Virginia and the e-Directive Registry: OLLI Course

The West Virginia Center for End-of-Life Care will be hosting a seminar via Zoom through OLLI (the Osher Lifelong Learning Institute at WVU) on July 23, 2020 at 12:30 PM.  Topics to be covered include: advance care planning in WV, the WV e-Directive Registry, and advance care planning during COVID-19.  Visit OLLI's website to learn how you can sign up to participate in this seminar.  This seminar is available to everyone but directed more for the public instead of health care professionals.

Completing the POST form: 2-part webinar series

The WV Center for End-of-Life Care is hosting a free 2-part webinar series on completing the POST form.  These webinars will be recorded and available for viewing through our website.  These webinars will be available for live participation for the first 100 participants.

The Physician Orders for Scope of Treatment (POST) form is a medical order form for patients with a serious illness.  The POST form is completed after discussion with the patient or incapacitated patient’s medical power of attorney representative or surrogate decision-maker regarding treatment preferences.  Once completed, in order to be valid, the physician must sign the form.  With the patient’s permission the form is submitted to the WV e-Directive Registry where it is available to treating health care providers 24 hours a day, 7 days a week.
