Make Your Treatment Wishes Known

The West Virginia Center for End-of-Life Care provides coordination, education, and resources so that at the end of life West Virginians will have their wishes for care known and respected.

for patients and families

For Patients & Loved Ones

Completing advance directives is the loving thing to do. Advance directives tell your loved ones what treatments you want when you are too sick to tell them yourself.

for healthcare providers

For Health Care Providers

Research shows that patients want their healthcare providers to talk to them about end-of-life care treatment options. We provide resources to facilitate that conversation.

What is the e-Directive Registry?

This electronic Registry houses and makes available to treating health care providers West Virginians’ advance directive forms, do not resuscitate (DNR) cards, and Physician Orders for Scope of Treatment (POST) forms.
