Health care professionals (clinicians) improve end-of-life care the most by talking with patients with serious illness and their families early in the treatment about the patient's goals for care.  Completing advance directives and/or medical orders (POST and DNR) is always voluntary.  These forms can be very beneficial in documenting patients' end of life care wishes, and the e-Directive Registry helps ensure these forms translate into patient care.

WV POST Program

The POST form is a medical order form for patients with a serious illness or advanced frailty.  The POST form is completed after a conversation with the patient (incapacitated patient’s medical power of attorney representative or surrogate decision-maker) regarding treatment preferences.  A copy of the form (front & back) should be submitted to the WV e-Directive Registry.

WV DNR card

The WV Do-Not-Resuscitate (DNR) card is a medical order that protects the rights of persons who do not want to have CPR in the event of cardiac or respiratory arrest.  The DNR card is completed by a health care professionals after discussion with the patient or incapacitated patient’s medical power of attorney representative or surrogate decision-maker.  A copy of the the whole (top & bottom portions) card should be submitted to the WV e-Directive Registry.

The WV e-Directive Registry makes individuals' documents readily available electronically.  Treating health care professionals can access Registry forms via the West Virginia Health Information Network's (WVHIN) online portal 24/7.  For additional information, click here.
